  I agree with the designer, style, color, and size for each item ordered. I understand that dresses are made as ordered, based on the size chosen. They are not custom made to measurements.
  Marie James does not perform alterations, and therefore, is not responsible for making any additional alterations.
  I understand that there is a 50% non-refundable deposit required on the full retail price on all special orders prior to ordering. All other items require payment in full.
  The balance is due and dresses must be picked up/shipped from Marie James within 14 calendar days upon receipt of the dress at Marie James. Customer will be notified of receipt by phone, mail, or email.
  Dresses will accrue a monthly storage charge of $15/ month if not picked up/shipped within the 14 calendar days
  Items left at Marie James 30 days beyond the stated event date become property of Marie James, Dress Boutique.

Once your order has been submitted, someone from Marie James will contact you for payment information. Please remember that bridesmaid orders will not be placed until the 50% deposit or full payment has been received. Please do not include credit card information on this form.
